Service Making Staff/Owner badges.

Hey, since I make staff badges "pretty" well, I thought I'd help people out by taking their requests for badges.
Basically, if you want a staff badge you can reply to this thread with a simple form, completed.
There are 2 staff badges which I'm currently making, let's call them design 1, and design 2.

Design 1:

You must be registered for see images attach

Design 2:

You must be registered for see images attach

Design 3:

You must be registered for see images attach

Now if you want a custom badge, please fill out the following form:

Hotel Name:
Title (Owner, Staff, Mod etc.):

I will reply whenever I've got time and have finished making them.

Okay, well I've made this badge, hopefully it's the right design as you haven't stated it yourself...
Unfortunately I cannot add snowflakes, sorry.
Next time please fill out the form correctly, as it only makes my work more difficult.
