1.Make new folder in your htdocs lets just call it badges.
2.Then paste the stuff from the rar file into the folder.
3.Upload the sql files which you should have pasted into the folder.
4.After this travel to it for example habtamz.sytes.net/badges or if that dosen't work habtamz.sytes.net/badges/index.php(Of course with your website.
5.Try to login if it dosen't work open up login.php from the badges folder and replace it with the code from the pastebin file.
6.If this works then you should just add a tab leading to it.

Did you upload sql and be more specific with the errors so I can help you. Post a screenie or somthing

No but you can code your its easy its should look like this :
