and it is a Contact Form, as the title explains it all.

As xeno said, does this submit to an email address? or text file?

Xeno, No, but if you wanted you could code it in or some Admin Panel.
Kaz, If I made it go to the email, with a different message the owner of website may not know how to change the message so I left it for people to add in.
Here it is with the submit button, you will have to do it so it goes to the email if you wish or somewhere.

Nice attempt, would be better if you had a nicer style and maybe made the "Question" field textarea rather than input.
