1. Are you database details correct?
2. Is your database updated / compatible with the version of Phoenix Emulator you are using? (On Otaku, when you download emulator, there is a file with SQL's in them. Use the most updated one)
3. Are you using a webhosts or xampp? Please not Phoenix doesn't work on webhosts.
