hey guys
i'm after a basic web form using bootstrap. i need to collect feedback which can be either logged into a basic database and displayed as a table on another page or form submits to email (might get my inbox spammed) and i manually update table records daily
ideally i want to be able to easily add/modify/remove fields. However, i want users to login using discord OAuth and auto fill username with their discord id/handle
any one down to do this, plus your charge [php/html]

- Dynamic form
- many inputs based on selected values (with text, dropdown and textarea)
- save to db and render in bootstrap 4 table
