Mod-Tools that disappears

Hey everyone, today I come back with a problem and this is the only problem I have not been able to solve ... I explain myself:

At any time, the mod-tools disappears (not when one is in the game, but when one disconnects, then reconnects), and when I restart, it comes back, but after it disappears, after a connection .. you have a solution?
Or do you have a command or something to make it reappear?

Seems like a packet error, if I'm not mistaken. Do you have an custom built Plus?
