Register for a specific Index page

I found a pretty good looking index page on here but it doesn't come with register. I could just pay someone to create me the index, but that's no good for me. Instead, is anyone willing to guide me through on how to code a register page for it? I can pay for that, I just don't want to be spoon fed.
